COVID-19 Liability Waiver

1. I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend wearing masks to protect myself and others.
2. I further acknowledge that PAPER STREET PLAY LLC has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.
3. I further acknowledge that PAPER STREET PLAY LLC can not guarantee that I will not become infected with the Coronavirus/Covid-19.

4. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to staff, and other customers and their families.
5. I voluntarily participate in activities as provided by PAPERstreet PLAY LLC and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19.

6. I acknowledge that I must comply with all set procedures to reduce the spread while attending my appointment.
7. I attest that:
* I am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
* I do not believe I have been exposed to someone or have a suspected and/or confirmed case of the Coronavirus/COVID-19.

* I understand that all of the statements above must be valid at check-in and these questions may be asked again for verbal confirmation

General Liability Waiver

I, as parent or legal guardian of the child whose name is set forth below (each referred to as a “Participant” and collectively referred to herein as the “Participants”) and in consideration of the Participants being permitted to participate in the Activities (as defined below) conducted by PAPER STREET PLAY LLC located at 40 Park Avenue #1, Park Ridge, NJ 07656 (“Paper Street Play”) I agree as follows:

  1. Activities – Activities shall be defined as the various activities offered to Participants by Paper Street Play.

  2. Assumption of Risk – I understand that the Activities entail the risk of severe bodily injury to the Participants. Injuries that could result may vary, but may include (a) minor injuries such as scratches, bruises and sprains; (b) major injuries such as eye injury or loss of sight, joint or back injuries and concussions; and catastrophic injuries, including paralysis and even death. I understand that, Paper Street Play cannot eliminate all risks or guarantee the safety of the Participants while Participants participate in the Activities. I have made my own investigation of these risks, understand these risks, and assume them knowingly and willingly on behalf of myself and my minor Participant child. Notwithstanding these risks and other hazards that may be foreseeable but not specifically identified herein, I, for myself and any minor Participant child, and our respective heirs, personal representatives and assigns, understand, acknowledge, and expressly and voluntarily assume all risks and full responsibility for any injury arising out of or related to the Activities.

  3. Release, Discharge and Agreement Not To Sue – I, for myself and any minor Participant child and our respective heirs, personal representatives and assigns, do hereby release, discharge and agree not to sue Paper Street Play or its managers, members, employees and/or other agents, for any injury to or death of any Participant arising, directly or indirectly, from participation in the Activities. This release, discharge and covenant not to sue shall relate to any and all claims or legal rights now existing or arising in the future, including claims and legal rights arising out of any negligence of Paper Street Play and/or its managers, members, employees and/or other agents and any other breach of a legal duty arising out of common law, statute, contract or otherwise.

  4. Indemnification And Release – I agree to indemnify Paper Street Play and release Paper Street Play from, without limitation, any and all claims, actions, suits, procedures, costs, expenses, damages and liabilities, including attorney’s fees and costs, incurred due to claims brought by any party as a result of or arising out of my or my minor Participant child’s involvement in the Activities and agree to reimburse Paper Street Play for any such costs, expenses and fees as they are incurred.

  5. Parent Or Legal Guardian Certification And Consent – I hereby certify that I am the parent, legal guardian of the minor Participant children whose name(s) appear below, and I have authority to waive rights on behalf of the minor Participant(s). I have read and I understand all of the provisions of this document and the risks of the Activities. I understand that the Activities could cause injury and even death. I acknowledge that I have read and understand the terms of this document and I am freely and voluntarily signing this document.

  6. I hereby grant Paper Street Play permission to use myself or my children in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration.

Notice of Rules 

  1. Unless otherwise noted, one responsible caregiver must be present at all times. One additional adult per admission is welcome for an additional $5 at check in. 

  2. For sessions where masks are required: A mask that covers your nose and mouth are required according to New Jersey guidelines. 

  3. Our facility is socks only. Adults and children are required to wear socks. We strongly suggest grippy socks, which may be available for purchase on site for $2/pair.

  4. Sanitizers are provided and required when enterting the play area.

  5. If a toy finds its way to a mouth, sneeze, or cough, please bring it to our “yucky bucket” located at the front desk.

  6. Waivers must be signed prior to play. 

  7. We recommend strollers stay in the car or at home. If you plan to bring one, please email or call ahead and we will do our best to accommodate stroller parking.

  8. We reserve the right to refuse service to any guest who is acting disorderly; including, but not limited to pushing, hitting, or other forms of aggression. The safety of our guests and staff is our priority.


While the state no longer requires masks or social distancing, we ask our guests to use kindness and consideration when joining us for play. Please respect personal space and call to reschedule when feeling unwell. Our refund policy can be found here.
